I, the duly authorized representative, hereby authorize U.S. South Communications, Inc, d.b.a. U.S. South and d.b.a. Interactive Communications International Inc. (?InComm?) to act as an agent for our Company in selecting, changing, and dealing with our local telephone company, long distance carrier, and Responsible Organization (as defined in the industry guidelines for 800 Number Administration) for the implementation of communication services and in particular to switch my/our company?s intrastate, interstate, and international long distance to U.S. South as my/our primary long distance carrier and my/our Responsible Organization to an entity to be designated by U.S. South. I state that I have the authority to authorize U.S. South to become the long distance carrier for the telephone numbers listed below.
I understand that the local phone company may charge a nominal fee to switch long distance carriers. I also understand that there may be only one primary long distance carrier for each presubscribed telephone number. I further understand that, in the future, if U.S. South changes underlying carriers, the local phone company may charge a switching fee. Should this occur, U.S. South will reimburse our Company for associated local phone company switching fees. U.S. South is authorized to take actions on my/our Company?s behalf to prevent unauthorized carrier switches including requesting the local phone company to implement a PIC freeze which precludes a charge in my/our Company?s long distance service without prior written instructions from my Company to our local phone company. I understand that any new 800 number(s) ordered are not guaranteed and should not be published until the new 800 numbers are actually working.
Each local phone company and long distance carrier is hereby instructed to make my/our Company?s customer service records and any other pertinent information available to U.S. South upon request of either U.S. South, myself, our Company. This letter of agency supersedes all previous letters of agency.
